What documentation do I need to submit to apply?

Just your signed application and 3 months of recent bank statements (4 months if your business is located in NY or CA). No tax returns, financial statements, or AR reports are required during application though they may be requested during underwriting.

Is there an interest rate?

No, there isn't an interest rate like a traditional loan. Instead, you receive a discounted amount of your future sales upfront, and you gradually repay the full funded amount over the term of the agreement. Do I pay the same amount every month?
